A 60-year-old man with diabetes presented to hospital with gradual onset hoarseness of 7 months duration. He was a smoker for the last 30 years and did not have any other symptoms such as cough, dyspnea or weight loss. Physical examination showed a normal pulse and blood pressure. Indirect laryngoscopy revealed a paralyzed left vocal cord in paramedian position (Picture 1). Thoracic aortic aneurysm presenting only as vocal cord paralysis.
We present a case of unilateral vocal cord paralysis in a 76-year-old Saudi male presenting with hoarseness of voice caused by thoracic aortic aneurysm. Here, we are illustrating a rare entity of thoracic aortic aneurysm presenting only with hoarseness of voice to draw attention to unusual clinical manifestations of this serious underlying aortic pathology.

The authors report the case of a 55-year-old female, hypertensive, smoker patient presenting with dysphonia, dysphagia and persistent dry cough. Laryngoscopy diagnosed left vocal cord paralysis. Computed tomography demonstrated saccular aneurysm of the inferior wall of the aortic arch, stretching the left recurrent laryngeal nerve, a finding compatible with Ortner's syndrome.
